Running mechanics do change across treadmill, road, trail, and track, but the size and coaching relevance of those changes are not equal. The strongest evidence indicates that motorized treadmill and flat overground running are broadly comparable, with small yet repeatable differences centered on sagittal-plane landing posture, knee motion during stance, center-of-mass displacement, and propulsion [3,4]. Track and firm overground running are also closer than many coaches assume, because runners rapidly adjust leg stiffness to preserve global running mechanics when surface stiffness changes [8,9]. By contrast, trail running is the most distinct condition: terrain irregularity and slope increase step-to-step variability, frontal-plane control demands, and metabolic cost [14–17]. For HYROX, this matters because the event is conducted indoors as eight 1-km runs alternated with eight work stations, and in the first peer-reviewed HYROX physiology study, running consumed the majority of total race time [1,2]. The practical hierarchy of specificity is therefore indoor overground first, then motorized treadmill for controlled load, with trail running used mainly to build robustness rather than direct race specificity.
Introduction
HYROX is not simply a fitness race with incidental running. Officially, the format consists of eight 1-km runs, each followed by one workout station, performed indoors in a fixed global format [1]. In the first scientific HYROX study, recreational athletes completed a simulated individual-open race in a median of 86.5 minutes, of which 51.2 minutes were spent running; faster overall times were associated with higher VO₂max, greater endurance-training volume, and lower body-fat percentage [2]. This shifts surface selection from a secondary programming detail to a central performance question for HYROX coaches.
Treadmill versus road: similar overall, not identical
The clearest evidence base concerns treadmill versus overground running. In a systematic review and meta-analysis of 33 crossover studies involving 494 participants, Van Hooren and colleagues concluded that most spatiotemporal, kinematic, kinetic, muscle-activity, and muscle–tendon variables are largely comparable between motorized treadmill and overground running [3]. The repeatable differences were small and mainly sagittal-plane in nature: treadmill running showed a less inclined foot–ground angle at initial contact, slightly more knee flexion at footstrike, a smaller knee-flexion range of motion during stance, lower vertical displacement of the center of mass or pelvis, slightly longer contact time, and slightly lower peak propulsive force [3]. Thus, treadmill running is not mechanically identical to road running, but it is also not a fundamentally different gait.
Physiological comparisons tell a similar story. A separate meta-analysis reported that motorized treadmill and overground running are generally similar for oxygen uptake and heart rate, although some differences appear across speeds and test formats [4]. Jones and Doust’s classic experiment remains relevant for coaching practice: a 1% treadmill grade most closely matched the energetic cost of outdoor running over approximately five minutes across a range of speeds, largely compensating for the lack of air resistance indoors [5]. However, energetic matching does not guarantee mechanical identity. A clinical study comparing treadmill video analysis with outdoor drone-based gait analysis found only moderate agreement for 2 of 8 kinematic variables and fair-to-poor agreement for the remaining 6 variables [6]. Likewise, curved non-motorized treadmills did not replicate overground joint kinematics better than standard motorized treadmills [7]. For HYROX coaches, the correct interpretation is that the motorized treadmill is a valid training and testing tool, but not a full replacement for indoor or outdoor overground running.
